Dayton, Ohio – The Dayton Fire Department is encouraging residents to take advantage of a free emergency preparedness tool designed to help first responders access important medical information during emergencies when every second matters.
City officials announced the continued availability of the “File for Life” program, a simple resource that allows residents to record critical personal and medical details on an easy-to-complete form.
The form includes information such as medications, medical conditions, allergies, and emergency contact information. Residents are instructed to place the completed form on their refrigerator so emergency responders can quickly locate it if they are called to the home.
Fire officials say the program can help save valuable time during medical emergencies by giving firefighters, paramedics, and other first responders immediate access to essential health information.
Program Supports Dayton’s Age Friendly Goals
According to city officials, the File for Life program is available free of charge to all Dayton residents.
The initiative is also connected to Dayton’s broader effort to remain an Age-Friendly City by helping residents of all ages stay safer and more independent.
Officials said the program is part of the larger Age-Friendly Dayton Plan, which focuses on improving quality of life, safety, and wellness throughout the community.
The city described the initiative as “A small step that makes a big difference.”
Residents can download the File for Life form and review additional safety resources through the city’s Fire Life Safety Resources program.
Officials also encouraged residents who need assistance with the form or program to call 937-333-5555 or 937-333-4520 for more information.
City leaders hope the resource encourages more families to prepare ahead of emergencies by making important medical details easily accessible for emergency personnel responding to homes throughout Dayton.
